The Kentucky Office of Vital Statistics collects, preserves and protects certificates for all births, deaths, marriages, divorces and stillbirths and issues certified copies.
Certificates may be ordered and obtained by the following methods:
Online: Certificates may be ordered online through VitalChek. You must have a major credit card to pay for the processing and mailing of the certificate when ordering online.
Telephone: You may request certificates by calling the VitalChek Call Center at 1-800-241-8322. You must have a major credit card to pay for the processing and mailing of the certificate when ordering by phone.
Mail: Orders are accepted by mail, using a check or money order (no cash) payable to the Kentucky State Treasurer. Mail payment along with the necessary information to:
